Louise R. (Urban) Slezak

SLEZAK – Louise R., 94, of Johnstown died December 6, 2018, at Conemaugh Memorial Medical Center. Born in Johnstown the daughter of Anthony and Apolonia (Grus) Urban. Preceded in death by her parents; husband, Alexander E. Slezak; infant son, Jacob; granddaughter, Katherine; brothers: Anthony, Joseph, Bernard, William, Alex, Frank and John; sisters, Genevieve, Stella, and Mary. Survived by her children: Theresa, wife of Earl Birch; Linda, wife of Don Thir; Patricia; Barbara, wife of Joseph Carrier; and Michael married to Louise (Lindemann); grandchildren: Kimberly, wife of Doug Shroyer; and Kevin Birch; great-grandchildren: Heather, Joshua, Allyson, Devin, and Jeremy. She was a member of St. Patrick Catholic Church. Louise will be remembered as a kind and loving wife, mother, grandmother, great-grandmother and aunt who will be missed by all. There will be no visitation for Louise however, family and friends are invited to attend a funeral Mass 1 p.m. Saturday at St. Patrick Catholic Church, Fr. Walter J. Moll officiating. Committal will take place at Benshoff Hill Cemetery. The family is being served by the Francis G. Ozog Funeral Home, Inc.
